Il medico-radiologo Savignano ricostruisce con efficacia, precisione e documentata competenza la trasformazione della sanità pubblica in Italia negli ultimi due secoli. Con lo sviluppo del «welfare» la medicina ha assunto il compito di promuovere la salute invece di limitarsi, ragionevolmente, a curare e a prevenire

Anche se il titolo principale non è che ci azzecchi con il contenuto, L’Anopheles sintetizza con efficacia, precisione e documentata competenza l’evoluzione della sanità pubblica in Italia negli ultimi due secoli. La forma letteraria nel titolo, che riflette la passione dell’autore medico-radiologo per le suggestioni impressionistiche indotte dall’arte medica, si riferisce alle zanzare che trasmettono la malaria, e quindi a una delle conquiste più significative della sanità pubblica nell’Italia unita, cioè l’eradicazione della malaria. Ma sulla malaria in Italia si dice l’essenziale.
Mentre il libro ricostruisce con ritmo incalzante e un buon gioco tra dimensione sincronica e diacronica dei problemi, colti in un’ottica non solo sanitaria, ma anche medica e quindi biopsicologica, le trasformazioni epocali della salute degli italiani negli ultimi due secoli.
Nonché l’evoluzione dell’organizzazione delle pratiche medico-sanitarie che hanno prima messo sotto controllo le principali malattie infettive, poi inseguito l’illusione politica che la salute possa mai diventare quel «benessere fisico, psichico e sociale completi» di cui dice all’articolo 1 la costituzione dell’Organizzazione Mondiale della Sanità del 1946. Il libro lavora su diversi registri storiografici: per circa metà, cioè fino all’indomani della Seconda guerra mondiale, riassume gli elementi salienti della letteratura storica, mentre gli ultimi cinquant’anni sono improntati a una cronaca di quel che è accaduto sul piano delle scelte di politica sanitaria. Il libro arriva quasi fino a oggi: nel senso che l’autore illustra la filosofia dell’attuale ministro Balduzzi, e solo per poco non fa in tempo a registrare l’allarme del presidente Mario Monti per la sostenibilità futura del nostro sistema sanitario, e in particolare per il suo impianto universalistico.
A chi si culla, abitando in case riscaldate, dotate di frigorifero e con nelle vicinanze supermercati, farmacie e ospedali, nel l’idea bucolica e pericolosa che nel passato si vivesse meglio di oggi, la lettura di questo libro andrebbe prescritta come terapia a tutela della salute pubblica. Perché predicare la decrescita, stramaledire il progresso tecnologico (salvando solo qualche balocco come internet e il telefonino), praticare il culto tribale dei cibi naturali e vagheggiare stupidamente di pericoli dovuti alle vaccinazioni e ai farmaci, o di vantaggi ottenibili usando medicine cosiddette alternative o anche naturali (omeopatia, erboristeria, psicoanalisi, eccetera) sono un viatico sicuro per tornare ai tempi in cui i bambini e le puerpere morivano come mosche, l’aspettativa di vita non arrivava a quaranta anni e le diseguaglianze economiche e sociali nell’accesso alle cure erano disgustose secondo i criteri di una morale umana anche minimamente altruista. Tempi non lontani come racconta Savignano. E quel che appare più disturbante nella vulgata tecnofobica e antiscientifica che ammorba i Paesi occidentali in declino, come l’Italia, è l’ingratitudine verso il lavoro degli scienziati e dei medici, da cui sono venuti i soli mezzi per tutelare finalmente e davvero la qualità della vita propria e delle persone che si amano.
Le riflessioni di Savignano non nascondono i problemi e il capitolo finale è un’ottima sintesi anche di questi. L’autore critica l’aziendalizzazione e le logiche econometriche che guidano la sanità pubblica, anche se il suo ragionamento scivola forse in modo troppo scontato verso i luoghi comuni. Ovvero vede le cause principali di alcuni squilibri fuori dal sistema sanitario, piuttosto che proprio dentro di esso e in una logica che, di fatto, sociologizza e politicizza la salute. Nel senso che nel momento in cui, con lo sviluppo del welfare state sanitario, la medicina è stata investita del compito di promuovere la salute, piuttosto che limitarsi, più ragionevolmente e nel rispetto di valori un po’ più liberali, a curare e prevenire le malattie, si è andati verso quella che il medico e scrittore inglese Michael Fitzpatrick ha chiamato «tirannia della salute». Nel senso che i medici occidentali, ispirati da una concezione sociologico-politica e davvero poco scientifica della medicina, hanno evoluto un nuovo paternalismo, sentendosi caricati del mandato morale e inquisitorio di diffondere una consapevolezza ossessiva dei rischi e promuovere un salutismo esasperato e costoso. E non c’è ragione di essere ottimisti per l’arrivo di una medicina, quella fondata sulla genomica, indubbiamente più scientifica in linea di principio. Ma anche già troppo piegata da logiche di facile consumo.
In questo quadro d’incertezza sarebbe “salutare” rileggere le pagine scritte nel lontano 1959 dal prestigioso batteriologo e grande intellettuale René Dubos in The mirage of health. Dubos svolgeva una spietata critica dell’idea positiva di salute, che stava per essere abbracciata dal welfare state sanitario.
Ragionando a partire da una visione evoluzionistica della salute e della malattia, visione che i medici faticano ovviamente a capire anche per il modo fuorviante in cui viene insegnata la medicina, Dubos prevedeva che l’aspettativa utopica di superare le sofferenze e la morte, inevitabili data la nostra biologia, avrebbe pesanti frustrazioni e alla lunga, per un’eterogenesi dei fini, un nuovo genere di danni. A cominciare da una crescita inarrestabile dei costi, prodotta da una domanda di salute in costante crescita e, di fatto incomprimibile dato che la salute è, appunto, un miraggio. Senza contare gli irresponsabili inviti a invecchiare il più possibile, senza dire che i vecchietti più o meno arzilli che vanno in televisione a dire cose quasi sempre intellettualmente penose, sono la punta di un iceberg che nasconde una popolazione in crescita di persone dementi e cronicamente sofferenti, che minaccia il dinamismo e l’efficienza, cioè la sopravvivenza di una società aperta al futuro. Curiosamente proprio quella che loro hanno costruito. Ma da giovani.
