Ma qualche passo verso una maggiore meritocrazia è stato fatto: dallo scorso gennaio tutte le nuove nomine dl direttori generali devono passare attraverso una preselezione. Nel servizio sanitario nazionale è allarme debiti e i manager della sanità sono sempre più nell’occhio del ciclone. Direttori generali, sanitari e amministrativi accusati da più parti di essere strapagati e nominati solo in base a criteri di lottizzazione politica e per nulla meritocratici. Ma loro non ci stanno: «Sostenere che oggi un manager della sanità è pagato troppo è avventato. Anzi, in molti casi ci troviamo di fronte a una difficoltà di reclutamento», osserva Valerio Fabio Alberti, presidente di una delle organizzazioni di categoria, la Fiaso.
È recente, ad esempio, il caso della Asl di Chieti che si è vista opporre una serie di rifiuti per il ruolo di direttore amministrativo perché lo stipendio in questione, ossia 90 mila curo l’anno lordi, veniva ritenuto troppo basso dai potenziali candidati. Attualmente, la retribuzione di un direttore generale non può essere superiore a 154mila euro (mentre i direttori sanitari e amministrativi percepiscono in busta paga il 20% in meno). Cifre in cui bisogna tener conto anche dell’effetto della cosiddetta legge Brunetta (1.133/2008) che ha previsto un taglio delle retribuzioni del 20%.
«La riduzione del trattamento economico è stata applicata in quasi tutte le regioni», precisa Alberti. «Con il risultato che la retribuzione media di un direttore generale è scesa a circa 123mila euro all’anno», mentre per direttori sanitari e amministrativi si è passati a circa 113 mila curo. In più può anche accadere che i dirigenti percepiscano meno dei loro sottoposti, a fronte però di responsabilità maggiori. «Confrontando la retribuzione di un capo dipartimento con quella di un direttore generale si scopre in molti casi che il primo ha uno stipendio maggiore rispetto al secondo».
Con conseguenze non di poco conto in termini di dignità e riconoscimento della carica. «A responsabilità elevata deve corrispondere una retribuzione proporzionale, altrimenti il rischio è di dequalificare il ruolo», commenta Alberti. A fare eco ad Alberti è Mario Del Vecchio, direttore del master in management della sanità dell’università Bocconi. «Nella Ausl di Bologna, una delle più grandi d’Italia, il direttore generale percepisce circa 150mila euro. Costi che, se una persona fa bene il proprio lavoro, sono risibili». Su questo fronte, da quando è iniziato il processo di aziendalizzazione della sanità pubblica, «il management è cresciuto trotto e occorre sempre discernere il tema dei manager da quello dalla sanità nel suo complesso, non incolpando i primi di tutti i problemi che hanno investito il settore», precisa Del Vecchio.
Anche secondo il presidente della Fiaso, in questi anni si è creata una comunità professionale di manager di buon livello. Ma «c’è bisogno di uno sforzo ulteriore in questo senso. Oggi, infatti, le risorse scarseggiano e, se con meno fondi si vogliono mantenere i servizi, la selezione di manager qualificati da parte delle regioni si rivela strategica per la tenuta del servizio sanitario nazionale». Con una parola d’ordine: nomine più libere e meritocratiche.
Queste ultime, infatti, «continuano a rispondere a criteri di lottizzazione politica», attacca Riccardo Cassi, presidente Cimo Asmd (Coordinamento italiano dei medici ospedalieri – Associazione sindacale dei medici dirigenti). «Certo ci sono dei bravi dirigenti che svolgono bene il proprio lavoro, ma non si possono chiamare veri manager, quanto piuttosto proiezioni della politica». Più cauto Del Vecchio: «Si tratta di figure che gestiscono l’80% del bilancio della regione, quindi chi gestisce i fondi pubblici non può essere scelto per concorso».
Qualche passo verso una maggior meritocrazia è stato comunque fatto: dallo scorso gennaio, infatti, tutte le nuove nomine di direttori generali devono passare attraverso una preselezione condotta da una commissione composta da esperti indipendenti e da un membro dell’Agenas (Agenzia nazionale per i servizi sanitari regionali). La rosa di candidati viene poi sottoposta alle regioni che provvedono a scegliere. La Regione Lazio ha, ad esempio, da poco annunciato la “rivoluzione del merito” che guiderà le assunzioni delle figure dirigenziali in ambito sanitario, con gli aspiranti manager che dovranno affrontare selezioni molto più rigide rispetto al passato. Resta infine aperto il tema della valutazione dei risultati «che oggi manca», sottolinea Cassi. «Per questo si potrebbe pensare a un ente terzo incaricato di elaborare un report dei risultati ottenuti dai manager».
