Per le Regioni in piano di il riparto 2013, ancora in stand by, si deve fare col nuovo modello dei costi standard solo per gli ultimi due mesi dell’anno. Per quelli precedenti l’idea è di usare il vecchio modello su cui i governatori lavorano di lapis per riequilibrare le risorse delle Regioni più in crisi. Per Salute ed Economia invece tutto l’anno dovrà essere passato al vaglio dei costi standard e di aggiustamenti ce ne potranno essere ben pochi.
Con il risultato che le Regioni più “vecchie” (Liguria e con tutta probabilità anche Basilicata) rischiano di perdere 50-60 milioni sui circa 200 che si sposterebbero tra una Regione e l’altra nel passaggio dal vecchio al nuovo sistema Di qui la necessità di operare alcune correzioni di rotta, utilizzando quote di premialità accantonate dagli esercizi precedenti. E questo perché anche le Regioni più ricche, che in questi anni hanno in parte ceduto qualcosa per quelle più in difficoltà, ormai non ce la fanno più a perdere quote di finanziamento. Fatto sta che la tabella promessa dal Governo non arriva ancora, così come i governatori ancora non mettono nero su bianco alcun numero, almeno finché non sarà chiaro il meccanismo finale. Un’attesa che andrà risolta al più presto però, anche perché le aziende sul territorio sono in sospeso per i loro bilanci sia consuntivi che preventivi per il prossimo anno. Su una cosa i governatori non transigono: dal 2014 il modello di benchmark va cambiato e vorrebbero farlo già nella legge di stabilità che questa settimana dovrebbe avere il via libera del Senato per poi tornare per il varo definitivo – senza modifiche ulteriori – alla Camera, inserendo un emendamento (al Dlgs 68/2011) ad hoc. Anche se i tempi non sembrano a loro favore. Così come il tempo che stringe verso fine anno non aiuta nemmeno a condurre in porto quel Patto per la salute promesso inizialmente entro Natale, ma che con tutta probabilità non si potrà chiudere se non a inizio del nuovo anno. L’ultimo modello di meccanismo di benchmark messo a punto dalle Regioni ricalca quello già presentato a suo tempo, al momento della predisposizione delle schede dei tavoli per il Patto sulla salute. Una proposta su cui tentennano le Regioni del Sud che vorrebbero ancora potere in qualche modo giocare la carta della deprivazione per alzare la posta del riparto. Il modello scritto in un vero e proprio emendamento alla legge di stabilità prevede ancora una volta l’allargamento del benchmark a tutte le Regioni senza piano di rientro e con i parametri in regola Poi, tra queste si deciderà quale prendere a riferimento. Gli standard medi nazionali che saranno fissati in base al loro modello dovranno essere rispettati da tutti e le Regioni in piano di rientro dovranno raggiungerli entro cinque anni. E il raggiungimento degli standard di qualità e di livello di servizio individuati annualmente sono anche la base per l’attribuzione alle Regioni, comprese quelle in piano di rientro, di elementi di premialità.
