di Elena Dusi, Repubblica. Quattro mesi potrebbero bastare per un vaccino aggiornato alle varianti. Due per riadattarlo in laboratorio. «E altri due per approvarlo» spiega Marco Cavaleri, responsabile vaccini e terapie per il Covid all’Ema, l’Agenzia europea del farmaco.
Le varianti sono rischiose?
«Sono insidiose sia per i vaccini che per gli anticorpi monoclonali. Ci diranno anche che direzione prenderà la pandemia. In Amazzonia sono partiti studi per capire se chi ha già incontrato il virus può riammalarsi con le varianti».
I vaccini funzioneranno meno?
«La variante inglese non sembra dare problemi. Diverso invece il discorso per i ceppi brasiliano e sudafricano.
Lì il potere neutralizzante cala con tutti i tipi di vaccino. Quelli a Rna partono da un livello alto e l’abbassamento non dà troppi problemi. Anche Novavax e Johnson&Johnson restano al di sopra del 50% di efficacia. Per AstraZeneca invece abbiamo visto ieri in uno studio in Sudafrica che la protezione si riduce quasi a zero».
Possiamo accontentarci così o già oggi c’è bisogno di rifare i vaccini?
«Anche se per molti vaccini la protezione resta sufficiente, tutte le aziende hanno iniziato a lavorare per coprire meglio le varianti. Noi dovremo decidere come autorizzare le nuove versioni. Serviranno dei requisiti di efficacia, ma senza allungarci troppo con i tempi».
A quali soluzioni pensate?
«Prevederemo piccoli studi clinici, con alcune centinaia di partecipanti, per capire il livello di risposta immunitaria. Non si tratterebbe delle grandi sperimentazioni che abbiamo visto finora, con tempi lunghi di attesa per calcolare i contagi, ma di semplici test per misurare gli anticorpi neutralizzanti. Ci vorrebbero almeno 4 mesi. Il vaccino riadattato potrebbe essere usato come richiamo, con una sola dose».
Quando finirà la carenza di dosi?
«Le cose non possono che migliorare. A metà marzo, se tutto andrà bene, dovremmo approvare il vaccino di Johnson&Johnson. Nel frattempo i vaccini già approvati, soprattutto Pfizer-BioNTech, avranno aumentato la capacità produttiva. Tra aprile e maggio, è atteso Novavax, che sarà seguito da CureVac e poi da altri».
Sarà possibile mescolare i prodotti? Fare la prima dose con un vaccino e la seconda con un altro?
«È possibile considerarlo. Oxford ha avviato uno studio che prevede la prima dose con il suo vaccino e la seconda con Pfizer-BioNTech e viceversa, ma auspichiamo iniziative simili. C’è motivo di credere che vaccini diversi possano anche offrire una protezione migliore».
L’alt di AstraZeneca in Sudafrica non rafforza l’idea che ci siano vaccini di serie A e di serie B?
«Può darsi che alcuni vaccini proteggano di più dalla malattia lieve, ma tutti danno un contributo importante nel proteggere dalla malattia grave e dal decesso. Su questo AstraZeneca dovrà affinare i dati, ma ha già dimostrato di avere un grande impatto. Il nostro obiettivo in questa fase d’emergenza è salvare vite e diminuire i ricoveri. Tutti i vaccini approvati sono utili per questo. Entro un mese vedremo dati chiari sul calo di decessi e ricoveri».
Resteremo sempre dipendenti dai vaccini?
«Non lo sappiamo. Dipende da come evolverà il virus. Può darsi che un ceppo si affermi sugli altri o che più varianti si ritrovino a convivere. È verosimile che i vaccini vadano ripetuti, ma forse non ogni anno».
Vari esperti chiedono lo Sputnik. Cosa sappiamo del vaccino russo?
«Abbiamo letto su The Lancet che ha un’efficacia superiore al 90%. L’Ema ha avviato un dialogo sotto forma di scientific advice. Non è ancora un processo di valutazione, ma una richiesta di informazioni. Non conosciamo i loro standard di fabbricazione, né la capacità produttiva, ma il dialogo va avanti».
Germania e Italia hanno deciso di usare gli anticorpi monoclonali come terapia. L’Ema cosa pensa?
«Cerchiamo di accelerare, ma servono più dati. Ci rendiamo conto della situazione di emergenza, per questo l’Ema fornirà un’“opinione scientifica” sugli anticorpi di Regeneron ed Eli Lilly. Non sarà un’autorizzazione formale — per quella non siamo pronti — ma una guida per fissare i criteri di somministrazione in emergenza».
Perché non siamo pronti?
«Non abbiamo riscontri consolidati per un’autorizzazione vera e propria. Sono prodotti non facili da usare Vanno somministrati in endovena a casa, in fase iniziale di contagio, ma con alto rischio di aggravamento. Li consiglieremo ad anziani e persone con altre patologie, per i quali saranno di grande aiuto».
