E’ stata pubblicata sul Bur numero 53 del 25 giugno la delibera numero 842 del 4 giugno con cui la giunta regionale ha approvato i Documenti di ricognizione delle prestazioni sanitarie Livelli essenziali di assistenza (Lea) assicurate dai Dipartimenti di Prevenzione delle Ulss del Veneto ai sensi del Dpcm 29 novembre 2001 e in attuazione del nuovo Pssr (legge regionale 23 del 2012). L’area di riferimento include la Sanità animale, la Tutela igienico-sanitaria degli alimenti di origine animale ed l’Igiene degli allevamenti e delle produzioni zootecniche. I documenti vengono ora inviati ai Dipartimenti di Prevenzione delle Ulss per la compilazione al fine di avere un quadro completo delle prestazioni Lea assicurate da ciascuna Azienda, con riferimento all’anno 2012.
Il nuovo piano socio sanitario 2012-2016 rivede i contenuti della programmazione sanitaria in relazione ai mutamenti dello scenario socio-demografico e al quadro epidemiologico. Ma anche in relazione ai nuovi bisogni assistenziali determinati dalla crescita dell’aspettativa di vita e dal progressivo invecchiamento della popolazione, alla prevenzione delle zoonosi e delle malattie emergenti; all’esigenza di perseguire stili di vita alimentari che contrastino i principali fattori di rischio di malattie; al rafforzamento della tutela del consumatore dalla contaminazione degli alimenti mediante adeguati controlli sanitari lungo tutta la filiera; all’assicurazione di un adeguato benessere animale.
I Documenti allegati verranno quindi inviati ai Dipartimenti di Prevenzione delle Ulss per la compilazione al fine di avere un quadro completo delle prestazioni Lea assicurate da ciascuna Azienda. L’incrocio poi del dato di attività complessiva, con le dotazioni organiche dei dipartimenti di prevenzione delle Ulss, permetterà di dare attuazione all’articoli 8 della legge regionale n. 23 del 2012 e quindi di definire le dotazioni organiche standard del personale sanitario, professionale e amministrativo necessarie a garantire l’erogazione dei Livelli essenziali di assistenza e i costi standard, per consentire il riparto delle risorse da destinare alle Ulss per l’erogazione dei Lea.
I cambiamenti economici, sociali, normativi, epidemiologici hanno determinato, nel corso degli anni, un aggiornamento dei contenuti delle prestazioni Lea, individuate dal decreto 29/11/2001, così che la Direzione Regionale Prevenzione e l’Unità di Progetto Veterinaria con i Dipartimenti di Prevenzione delle Ulss del Veneto hanno avviato un complesso lavoro di ricostruzione e di aggiornamento delle prestazioni LEA, erogate dai Dipartimenti di Prevenzione delle Aziende ULSS, elaborando un nuovo documento di ricognizione.
Per quanto riguarda l’area di Sanità pubblica veterinaria e di Sicurezza alimentare, i principali livelli essenziali di assistenza sono mutuati dagli obiettivi stabiliti di recente dall’Unione Europea con lo scopo di assicurare un alto livello di sicurezza alimentare, salute e benessere degli animali, attraverso l’adozione di una strategia stabilita a livello comunitario e definita “dal campo alla tavola” che copra tutti i settori della catena alimentare, inclusa la produzione di mangimi, la produzione primaria di alimenti, la loro lavorazione, immagazzinamento, trasporto e commercio, inclusa la vendita al dettaglio.
In particolare, per quanto riguarda la Sanità Animale uno dei compiti prioritari dei Servizi Veterinari è rappresentato dalla prevenzione e controllo delle malattie infettive trasmissibili dall’animale all’uomo (zoonosi). Recentemente si è affermata la consapevolezza dell’impatto economico che le malattie infettive animali, come l’influenza aviaria e l’afta epizootica, possono avere per gli allevatori e di conseguenza dell’importanza delle azioni poste a difesa del patrimonio zootecnico.
La gestione dei piani di risanamento ha rappresentato la prima fase di attività che ha consentito di raggiungere la qualifica di Regione Ufficialmente indenne per leucosi e brucellosi bovina e ovicaprina e per tubercolosi bovina. La diretta conseguenza di tali riconoscimenti è rappresentata dalla possibilità di modulare la frequenza dei controlli negli allevamenti, sulla base di quanto disposto della Direttiva 64/432/CEE, coniugando gli obiettivi di risparmio della spesa pubblica con quelli di miglioramento dell’attività di controllo. Successivamente sono stati stabiliti specifici piani di monitoraggio e controllo per le salmonellosi, le TSE (BSE e Scrapie), le pesti suine, la Malattia Vescicolare Suina e la malattia di Aujeszky.
Con il fenomeno del cambiamento climatico (surriscaldamento) si è assistito alla comparsa di malattie esotiche sostenute da vettori. Tra queste il controllo della West Nile Disease ha valenza di monitoraggio della circolazione virale al fine della prevenzione di questa importante zoonosi. Il piano di controllo della Blue Tongue invece viene effettuato soprattutto per l’importanza economica della malattia e per la movimentazione di animali.
Per il controllo delle malattie infettive particolare importanza viene posta alla verifica delle misure di biosicurezza, atte ad impedire o limitare l’insorgenza di tali patologie e i controlli per l’anagrafe zootecnica che garantiscono la tracciabilità delle movimentazioni necessarie (il tracing back) in caso di focolaio.
La cura delle malattie degli animali riconosce nell’utilizzo del farmaco veterinario uno strumento indispensabile per il ripristino delle condizioni fisiologiche e per il benessere degli animali. Mentre per la cura delle patologie degli animali d’affezione l’attenzione viene rivolta essenzialmente all’efficacia del principio attivo utilizzato, nelle terapie per gli animali destinati a produrre alimenti o ad essere essi stessi alimenti per l’uomo, vanno tenuti in grande considerazione gli aspetti relativi alla somministrazione illecita di sostanze vietate e alla somministrazione abusiva di sostanze autorizzate, nonché alla verifica della conformità dei residui di medicinali veterinari con i limiti massimi di residui fissati dal regolamento (CE) n.37/2010. A tal proposito il Ministero della Salute prevede annualmente l’applicazione di un Piano Nazionale Residui (PNR) con l’obiettivo di evidenziare non conformità nell’utilizzo dei farmaci veterinari.
Per quanto riguarda l’alimentazione animale il Piano Nazionale Alimentazione Animale assicura, in accordo con quanto introdotto dal Regolamento (CE) n. 178/2002 e dal Regolamento (CE) 882/2004, un sistema ufficiale di controllo dei mangimi lungo tutta la filiera dell’alimentazione animale al fine di garantire un elevato livello di protezione della salute umana, animale e dell’ambiente. In particolare il Regolamento (CE) n. 882/2004 prevede che i controlli siano effettuati periodicamente, con frequenza appropriata, in base alla valutazione dei rischi tenendo conto del numero e della tipologia di aziende del settore dei mangimi e della specie animale di destinazione dei mangimi, delle caratteristiche e dell’uso dei mangimi o di qualsiasi trasformazione, attività, operazione che possa influire la sicurezza dei mangimi.
Nel campo della Sicurezza alimentare si sta sempre più consolidando, per prevenire le patologie cronico-degenerative e le loro complicanze, la promozione di stili di vita sani, legati ad una corretta alimentazione atta ad evitare anche l’obesità, con particolare riguardo a quella infantile, e la riduzione del numero di consumatori di alcol a rischio, attuata con Programmi e Progetti di formazione ed informazione che consentono di attuare il cambiamento nel modo di concepire la prevenzione e nelle strategie adottate in ambito preventivo.
Nel campo della prevenzione delle malattie determinate dagli alimenti, inclusa l’acqua, la recente normativa sanitaria dell’UE, conglobata per gran parte nel cosiddetto “Pacchetto Igiene”, ha segnato una svolta nella programmazione ed implementazione dei controlli ufficiali sugli operatori del settore alimentare, così come previsto dal Regolamento CE n. 882/2004, effettuati sulla base dell’analisi del rischio, con l’esecuzione di controlli innovativi quali l’attività di audit di settore, introducendo anche l’Audit atto a verificare l’efficacia del sistema di controllo posto in essere dalle Autorità competenti designate.
Le azioni a tutela del consumatore sono state negli anni ridefinite sulla base di emergenze ed innovazioni, tra le quali le più note: il pericolo di assumere parassiti con i prodotti della pesca consumati crudi; l’aumento del rischio di contaminazione degli alimenti con sostanze chimiche di origine ambientale, dall’uso di anabolizzanti e farmaci o di origine fraudolenta, quali lo scorretto utilizzo di additivi ed integratori; la necessità di garantire la sicurezza igienico sanitaria dei prodotti provenienti dalle piccole produzioni locali tramite la definizione di un modello di standard igienico-sanitario per le filiere fragili; le problematiche inerenti la rintracciabilità e l’etichettatura dei prodotti alimentari.
Assume poi sempre più importanza l’azione di controllo sul benessere animale, con la recente normativa introdotta dalla UE, che consente indiscutibili benefici agli animali, ma anche di elevare, in ultima analisi, la qualità degli alimenti di origine animale e loro derivati ricavati dagli stessi animali.
Cambiamenti importanti sull’attività quotidiana, con evidenti miglioramenti di tipo gestionale, sono poi intervenuti in questi ultimi anni grazie al potenziamento del sistema informatico di prevenzione finalizzato ad una gestione coordinata delle attività, anche ai fini del monitoraggio LEA, all’avvio di Piani integrati di controllo, di monitoraggio e di campionamento, all’utilizzo di tecnologie e di strumenti telematici innovativi nella formazione degli operatori sanitari e all’avvio di un sistema di comunicazione per garantire una corretta informazione del cittadino, del consumatore finale, degli operatori del settore e delle associazioni di categoria.
L’Allegato “B” al provvedimento, di cui costituisce parte integrante, individua, per l’Area di Sanità Pubblica Veterinaria e Sicurezza Alimentare, per gli ambiti relativi a Igiene degli alimenti e nutrizione, Sanità animale, Tutela igienico-sanitaria degli alimenti di origine animale, Igiene degli allevamenti e delle produzioni zootecniche, le attività e le relative prestazioni LEA, secondo indicatori e standard di riferimento.
