Gli Stati Uniti e altri 31 paesi sanno chi dovranno incontrare nel girone dei Mondiali di calcio 2014 che si disputeranno in Brasile a giugno e luglio. Le squadre e i tifosi sanno anche in quali dei 12 stadi disseminati nel paese più grande del Sudamerica giocheranno le loro partite.Proprio in quel periodo, in tre delle città ospitanti – Fortaleza, Natal e Salvador, tutte nel Brasile nordorientale – la stagione della dengue sarà vicina al picco infettivo. Il Brasile, infatti, è una zona calda per questa malattia tropicale, un virus trasmesso dalle zanzare per il quale non esiste un vaccino o una cura e che si sta diffondendo sempre più. In Brasile i morti per dengue nel 2013 sono stati 573, quasi il doppio rispetto al 2012. Molte persone guariscono senza complicazioni, ma nella forma più grave la dengue uccide un malato su quattro.
“Si sta verificando un costante aumento della dengue in gran parte del mondo, specialmente in Sudamerica”, dichiara John Brownstein, pediatra e ricercatore al Children’s Hospital di Boston.
Secondo una ricerca condotta da Brownstein e altri suoi colleghi, si stima che il numero di casi mondiali sia arrivato a 390 milioni l’anno.
L’urbanizzazione, la mobilità e i mutamenti climatici possono contribuire all’impennata della malattia, e tutti questi fattori potrebbero aumentare i rischi di trasmissione durante i Mondiali dell’estate prossima. Tuttavia gli esperti auspicano che, richiamando l’attenzione sul problema, aumenterà la consapevolezza e si prenderanno provvedimenti più incisivi contro questa malattia che tende a essere offuscata dalla malaria.
“Il Brasile è in una posizione di forza per guidare una battaglia internazionale contro la dengue”, afferma Simon Hay, infettivologo dell’Università di Oxford, che ha messo in guardia contro i rischi della malattia in un articolo apparso su “Nature”.
In previsione di due grandi manifestazioni sportive internazionali – il Brasile ospiterà anche le Olimpiadi estive del 2016 – il paese sta costruendo nuovi stadi e sta dotando di nuove infrastrutture molte sue città. Hay segnala che per i Mondiali ci si aspetta di vendere 3 milioni di biglietti.
Le zanzare Aedes aegypti, che sono i principali vettori della dengue, “amano l’ambiente umano”, dichiara Cory Morin, ricercatore associato in geografia e sviluppo all’Università dell’Arizona. “Per queste zanzare, più l’area è urbanizzata, meglio è.”
Così come l’assembramento di migliaia di tifosi può favorire la trasmissione, l’urbanizzazione implica che le zanzare hanno più persone da pungere. Significa anche che ci sono più pneumatici, giocattoli, fioriere, impervi cantieri edilizi che trattengono l’acqua e offrono un habitat perfetto per la riproduzione delle zanzare.
“Se verranno attuati molti interventi sul paesaggio, potrebbero crearsi altri habitat riproduttivi in aree vicine agli stadi”, afferma Shannon LaDeau del Cary Institute of Ecosystem Studies di Millbrook, N.Y. LaDeau segnala che i giardini e i rifiuti causati da grandi assembramenti di persone potrebbero acuire i problemi in Brasile. “Se ci sono pioggia e rifiuti, allora c’è l’habitat per le zanzare.”
“Magari questa attenzione dovuta ai mondiali sarà un grande incentivo per attuare cambiamenti a lungo termine”, aggiunge LaDeau. “Di certo mi auguro che qualunque sviluppo venga attuato non aggravi i problemi del Brasile.”
Hay raccomanda alle autorità brasiliane di adottare misure “aggressive” per il controllo delle zanzare prima che inizi la stagione della dengue e si disputi il torneo. Le misure comprendono lo sgombero dei “rifiuti” e l’utilizzo di insetticidi quando si individua la presenza di zanzare adulte. Ai tifosi che assisteranno ai Mondiali, Hay suggerisce di scegliere alloggi in spazi chiusi dotati di zanzariere e aria condizionata, di usare repellenti antizanzare e indossare abiti che coprano braccia e gambe. Hay e altri esperti sottolineano l’importanza di informare le persone e promuovere la prevenzione individuale.
“Ci sono persone che arrivano da luoghi in cui magari non hanno mai sperimentato i reali problemi causati da malattie trasmesse dalle zanzare”, spiega Brownstein. “Magari non sono sensibilizzate verso questo rischio.”
Una rapida occhiata alla Mappa della dengue può aiutare i viaggiatori a visualizzare la portata del pericolo attuale. I punti rossi indicano la presenza della malattia.
Brownstein ha contribuito a creare diverse mappe delle malattie infettive, tra cui questo strumento visivo che attinge a banche dati ufficiali, quotidiani, social media e altre fonti per fornire stime aggiornate sui pericoli della dengue in tutto il mondo. Ha dichiarato che durante il torneo di calcio la sua équipe terrà monitorato il Brasile e i paesi collegati tramite le vie di comunicazione.
Un altro motivo di preoccupazione è che i tifosi che assisteranno ai mondiali potrebbero portare a casa il virus. Molti dei casi di dengue che si sono manifestati negli Stati Uniti sono stati importati da viaggiatori rientrati da zone tropicali. Nel decorso della dengue può passare una settimana circa dalla puntura di una zanzara al momento in cui compaiono la tipica febbre alta e i forti dolori dietro gli occhi e alle articolazioni. Se in molti casi il problema si risolve nell’arco di pochi giorni, in una piccola percentuale la dengue si può trasformare in una febbre emorragica potenzialmente letale.
Più del 40 per cento del mondo è già a rischio.
I paesi sviluppati sono riusciti a combattere la dengue molto meglio del resto del mondo, specialmente grazie all’aria condizionata, l’accesso alle cure sanitarie, i sistemi di drenaggio dell’acqua piovana e abitazioni ben protette. Eppure gli esperti avvertono che paesi come gli Stati Uniti non sono immuni. Se è vero che i casi di dengue importata causano raramente focolai epidemici, i fatti dimostrano che la dengue potrebbe riprendere ad attecchire inFlorida e in Texas dopo uno iato di diversi decenni.
Secondo alcuni esperti, i mutamenti climatici sono tra i fattori che rendono alcune zone degli Stati Uniti più accoglienti per la zanzara Aedes aegypti, che normalmente abita in zone tropicali. Quest’estate l’invasiva Aedes aegypti ha fatto la sua comparsa nella California del nord.
Anche la zanzara tigre asiatica, un’altra specie invasiva sempre più diffusa negli Stati Uniti, si è dimostrata capace di trasmettere la dengue. Anche la zanzara tigre sembra espandere il suo habitat a nord per via del riscaldamento globale.
Morin, dell’Università dell’Arizona, è l’autore principale di uno studio pubblicato di recente sui complessi rapporti tra i mutamenti climatici e la dengue.
Le temperature più elevate, spiega Morin, possono accelerare passaggi cruciali che portano alla diffusione della malattia: incremento della popolazione di zanzare, replicazione del virus nella zanzara e riduzione dell’intervallo tra il momento in cui succhia sangue da una persona e il momento in cui può trasmettere il virus a un’altra persona.
Tuttavia, il calore intenso e un tasso di umidità ridotto possono rendere la vita più difficile alle zanzare, aggiunge Morin. L’interazione tra mutamenti climatici, temperatura e precipitazioni è complesso.
In linea generale, prosegue Morin, i mutamenti climatici potrebbero avere minore effetto sul Brasile che sugli Stati Uniti. “Il Brasile ha già temperature ideali”, dichiara. “Ma la Florida meridionale potrebbe essere al limite della soglia di trasmissione, perciò anche un lieve aumento delle temperature potrebbe avere ripercussioni notevoli.”
I brasiliani hanno già organizzato diverse proteste contro la decisione del governo di spendere miliardi di dollari per ospitare i Mondiali di calcio e le Olimpiadi. Molti cittadini credono che il denaro dovrebbe essere speso per alleviare la povertà e per migliorare il sistema sanitario e le infrastrutture pubbliche, investimenti che potrebbero avere un ruolo cruciale nel combattere la dengue.
Hay ha dichiarato all’HuffPost che né il governo brasiliano né la FIFA, l’organizzazione che sovrintende ai Mondiali di calcio, hanno risposto al suo articolo. Ha aggiunto che spera che l’articolo possa stimolare il dibattito.
In un comunicato via mail, un portavoce della FIFA ha dichiarato all’Huffington Post che “la FIFA e il Comitato organizzativo locale sono consapevoli dei rischi e dei timori per la febbre dengue in Brasile” e che questo tema “sarà una delle questioni sanitarie che saranno presentate alle squadre e alle delegazioni prima che venga disputata la Coppa del Mondo.”
“Il Brasile è solo uno dei cento paesi che combattono contro la piaga sempre più diffusa della dengue”, ha aggiunto Hay. “Portare in primo piano questo problema, nel contesto dei Mondiali di calcio, rappresenta una buona occasione per invocare un’azione più coordinata e internazionale contro questa malattia tropicale che va diffondendosi.”
